Reports

Reports are typically used to communicate and summarise results of formalised assessments. Reports may be needed in a variety of instances including: applications for various funding schemes, scholarships, and selective placements including:

  • Disability support pension applications

  • Housing commission applications

  • NDIS funding applications

  • Selective placement applications 

    • Gifted classes​

    • Performing arts scholarships

    • MENSA

Listed below are some of the common reports produced by our psychologists.

Behaviour support plans aim to communicate strategies to key support people in relation to reducing behaviours that may be harmful or disruptive and replacing them with more functional alternatives. 

Diagnostic Formulation

A diagnostic formulation is used to communicate results from clinical interview and observation, as well as evidence based assessments relating to specific psychiatric diagnoses. 

NDIS Plan Review Report

NDIS participants are recommended to have supporting reports by therapists at their review meetings in order to ensure their subsequent plan is appropriate for their individual needs.

School Report

Schools often request reports from a psychologist to gain information in relation to behavioural, academic, or mental health matters. 

Court Report

In some legal proceedings, such as financial, criminal, and divorce cases, expert opinion reports may be sought by lawyers to gain information on factors such as the impact of mental health on matters relevant to the case.

NDIS Application Report

The NDIS requires supporting documentation in applications to become and NDIS participant. In many cases, a psychologist can provide this documentation via a report.

Psychometric Report

A psychometric report is used to communicate the results from IQ or academic achievement tests which may be relevant to schools, funding schemes, or other selective programs.

Treatment Plan

Our psychologists tend to provide a treatment plan to patients, parents, and other support people where relevant, to align goals and intervention strategies.
